Can the October 2011 deadline be extended for the requirement for remote control timers or lockout devices?
Yes, but only with a formal variance from the Chautauqua County Health Department. Any variance request must include an abatement schedule and alternate provisions to protect the safety and health of the patrons.
Can the operator develop their own statement of acknowledgement or must the DOH form be used?
No, the DOH form must be used. However, it can be put on card-stock or half-sized, as long as it remains readable.
Does the remote timer have to be outside of the booth or room in which the bed is located, such as on the exterior side of the door of the booth?
Yes, providing the timer is not under the control of the patron.
If manual shut off is within the room for the bed or booth, is that considered the immediate environs (area) of the UV device?
Yes
If there is no manual shut off within the room, can the machine still be used?
No, a manual shut off is required.
The concentration of the disinfectant, expressed as parts per million (ppm), is not always listed on disinfectant containers. How does facility operator determine the required concentration?
The operator should contact the manufacturer to get information on concentration and contact time.
What is a lockout device?
It is a device that allows operators to lock and unlock the timer by way of punching in a code or similar method so that a patron cannot set or reset their own time.
Will previously completed patron paperwork (consent form, statement of acknowledgement) be acceptable?
No, the operator should have patrons fill out the new Subpart 72-1 complaint paperwork on their next visit.
