County Absentee Ballot Application

The Governor has signed an Executive Order (EO 202.15) that due to the potential for every voter to be a carrier of the COVID-19 virus, "temporary illness of physical disability" would be the reason on the application to select for voters who wish to file an Absentee Ballot Application for the COVID-19 Pandemic.

***** Just for the June 23, 2020 Primary, the N.Y.S. Governor (EO 202.15) has allowed for the requesting of an Absentee Ballot via e-mail or fax, without the need for a signature. Please provide your Name, Address and Date of Birth in your request for us to be able to confirm your identity. Your signature is still required on the envelope before the return of the Absentee Ballot. Send E-Mails to vote@co.chautauqua.ny.us or Fax at 716-753-4111.

***** At this time, applications for any Election occurring after June 23, 2020 are still required to send the physical copy of one of the below applications with your original "wet" signature either via the United States Postal Service or by dropping the application off in person at the Board of Elections Office.

Absentee Voting can take place any time at the Board of Elections Office during all business hours. Print and mail applications to the Board of Elections, 7 N. Erie St. Mayville, NY 14757 and the Ballot will be mailed the same day that the application is received. Board of Elections mail CANNOT be forwarded and Ballots will be delivered to the address directed by the Voter on the Absentee Ballot Application.
