Why Your Feedback Is Important
The Chautauqua County Community Health Assessment (CHA) uses public health data, community surveys and focus groups to identify the health needs and issues of the county.
The Chautauqua County Community Health Improvement Plan (CHIP) is the long-term effort to address some of the public health problems that are identified through the CHA process and activities.
Together, the CHA and CHIP summarize the information about the county’s current health status, needs, and issues and then plan where resources should be used to best meet the community’s needs.
The CHA and CHIP can be used as resources for health care providers, policy makers, social service agencies, community groups/organizations, religious institutions, educational institutions, businesses, and residents who are interested in working toward improving the health status of Chautauqua County and the overall quality of life of its residents. It is the intent of the Chautauqua County Healthy Department and its community partners that the information presented in this report is a useful community resource and a catalyst for new ideas and effective collaborations, interventions, and campaigns.
